At Goldilocks Community Health, we successfully navigate and support participants along their own journey every step of the way. We are a team of likeminded passionate Health Professionals with extensive experience in a range of Inpatient and Community service settings. Our established networks with local Service Providers have ensured quick, easy, and smooth engagements so that our participants optimise their NDIS funding and exercise choice and control. We are dedicated to building independence and maintaining everyone's overall health and wellbeing, so our team is absolutely focused on participant centered care by offering services that address all aspects of wellness.

Support Coordination and Recovery Coaching
If you need Support Coordination then as a registered provider we are here to ensure you are supported with your plan and service engagements from beginning to end. Respite / STA Options
As a Registered NDIS provider we encourage self-directed respite options so our participants can collaboratively design their supports (using our team or their existing teams), The NDIS list of supports as stated in your plan will be 100% compliant with reporting completed by our senior Care Manager every stay - so please complete the expression of interest below so we can create together.

CPD and Supervision for capacity buidling life game changers are what makes the health industry innovative and majestic. Come and join a collaboration of health professionals who practice at the highest performance levels - learning ways to improve how we write NDIS reports fulfilling the funder requirements, being able to empower participants with a schedule of support using the providers they chose, and grow as an individual practitioners, to pave the way for our communities - join us by completing an expression of interest below as we offer packages of learning mid May 2025 onwards - WATCH THIS SPACE

We are SLES, Employment and Specialist Employment registered NDIS providers. We assist with Job Capacity assessment and outcomes every step of the way. We link you to local DES/IEA providers who are part of your team that engages work you chose, are skilled and supported to engage in - so please book a free 30 minute planning session with our team using the expression of interest form below
Specialist Support Coordination - Home and Living Assessments
Goldilocks Community Health is a registered Level 3 Support Coordinator provider specialising in complex needs supports and home and living assessments through evidential practices - SILS, SDA, ILO, MTA and STA. Functional Capacity Provider Partners enhance outcomes with Short or Long term projects mapped out to achieve your existing or proposed goal outcomes so please contact us for a free initial consultation as our 100% funding outcome results for all participants over 5 years will be hard to find!
